Terms & Policies
RESERVATION POLICIES:
Reservations for services should be made at least 24 hours in advance. In the event of an emergency, dial 911.
CANCELLATION POLICY:
If you fail to give 24 hour notice of a cancellation or rescheduled appointment, a fee will be charged to your account. If for any reason your account is sent to a collection agency, it is the policy of this office to charge for any cost incurred in collecting past due or delinquent charges.
CHECK-IN:
Please arrive at least 10 minutes before your appointment time.
MEDICAL CONCERNS:
A list of prescriptions being taken at the time of service must be supplied. All medical concerns should be documented during each appointment.
